Go to the Page Layout tab

Click Margins and select Custom Margins.

Go to the Insert tab

Click on Table

Select Insert Table

Enter 2 for columns, 31 for rows

Autofit to Window

Click Here

Select the entire table (you can click the little select handle on the upper left side)

Right-click the table and go to Borders and Shading

Click 'None' to remove the existing borders

Select the horizontal lines and only the middle vertical line

Click OK

Right-click the table again and go to Table Properties

Select Specify Height

Enter 0.8

Change the Row Height Is to Exactly

Click OK

Select just the first column in the table.

Right-click and go back to Table Properties

On the Column tab, change the Preferred Width to 1.5 and select centimeters in the Measure With box.

Click OK.
